{{edit COI}} Is there a proper template for that?

Can someone please tell me the contents/wikitext of the latest version of this page before it was deleted? Due to a technical glitch at Wikikimedia Commons (where the file now resides), the file description got lost and we need to restore the license. -- Rillke (talk) 11:13, 5 February 2013 (UTC)

What's the issue here? I would like to get the old file description. I am administrator at Wikimedia Commons and we need it. Otherwise we have to delete the file because it is leaking one essential information. -- Rillke (talk) 22:48, 9 February 2013 (UTC)

I don't see any deleted material here after looking, but I'm not a file specialist and this is the first time I've had this kind of request, so I might be missing something.
